Happy Hollow School Forms
The following forms are required and must be completed for all students attending Happy Hollow School and must be returned no later than August 1, 2010.
- Developmental History and Background Information
- Family Information and Email Address Contact List
- Transportation Plan/Pick-up & Release Permission/Emergency Transport Permission
- First Aid & Emergency Medical Care Consent Form
- Consent Form
- Physician Statement Form
(Physicians may use their own form(s) if all information is included. Please note that a copy of your child’s immunization record must also be included.)
The following forms must be completed if applicable to your child’s specific circumstance:
- Notice of Allergy
(This form is required if your child has an allergy. Please be sure to thoroughly complete the form and have your child’s physician complete and sign the lower portion. Please list only one allergy per form.) - Medication Consent Form
(This form must be completed if Happy Hollow School will be administering any medication, including emergency medication such as Epi-Pens or Benadryl. Please note that a physician’s signature is required for prescription and non-prescription medication. A separate form is required for each medication.) - Reciprocal Consent to Release and Share Information
(Please complete this form if your child is working with a specialist and you wish that specialist to communicate with our staff, i.e. speech therapist.)
